Even though I hadn't really played on my alts by that time, I went back and rechecked all of the 44 fragments that I could, but I didn't find any that I missed. I checked destinytracker and it said I had 45 but the in-game Dreadnaught map showed I only had 44. I quickly ran the week 2 T3 CoO and got my 45th fragment. In my case, I had obtained 44 fragments by week two and didn't have the 3 fragments from T3 CoO and the 3 other fragments that haven't been found yet.
